THE RHIZARTHROSE
 
 
WHAT IS THE RHIZARTHROSE?

The rhizarthrose is the osteoarthritis of the base of the thumb. It reaches the articulation between the trapezoid and the first metacarpal.


WHAT ARE THE SYMPTOMS OF THE RHIZARTHROSE?

The usual symptom is the pain, particularly in the movements of everyday life using the pollici-digital grip (i.e. the thumb with another finger) like turning a key, opening a bottle, or a window.
Little by little, the articulation worsens then dislocates and then appears a distortion characteristic at the base of the thumb.


WHICH COMPLEMENTARY EXAMINATIONS ARE USEFUL?

Radiographies of face and profile of the thumb (incidence of Kapandji) make it possible to confirm the clinical diagnosis and to appreciate the importance of the articular destruction and the conservation of a certain osseous volume.


WHAT IS The TREATMENT OF The OSTEOARTHRITIS OF The BASE OF The THUMB?

If a preserving treatment well-led for 6 months to 1 year remains inadequate, a surgical operation can be considered. Two great types of interventions are proposed according to the age, the destruction of the articulation, work and the dominating side (right-handed or left-handed person).

The trapezectomy consists in removing the sick bone. To stabilize the thumb we generally associate a ligamentoplastic surgery intervention to the same finger, i.e. the use of a tendon of the vicinity which is threaded at the base of the first metacarpal to make it stable.

The trapezium-metacarpal prosthesis. This prosthesis resembles a little prosthesis of the hip, with a spherical metal head which is articulated in a trapezium cupula out of polyethylene or metal.


WHAT IS THE EVOLUTION AFTER The SURGICAL TREATMENT?

You will leave the private clinic 24 to 48 hours after the intervention, with a splint maintaining the thumb "isolated" in order to avoid the pains and to facilitate the cicatrisation. You will return in consultation with your surgeon on the 3rd day to change your bandage and to check the scar.

Rehabilitation will begin more or less early according to the type of intervention:

Since the 8th day in the event of prosthesis
After 4 or 5 weeks in the event of trapezectomy with tendinous interposition. A complete recovery can require 3 to 6 months.


THE DURATION OF THE STAY IN TUNISIA:

A hospitalization of 24 to 48 hours is to be considered then a 2 weeks convalescence on the whole at the hotel in half board.


WHAT ARE THE SPECIFIC RISKS?

The risks are almost the same whatever the technique: irritation of the cutaneous nerves, infection, unsealing of the prosthesis. These problems are exceptional and generally do not leave any after-effect if they are treated in due time.
